Insurance and Fee schedule

INSURANCE COVERAGE

We accept Anthem/Blue Cross and Blue Shield, Cigna Healthcare, Harvard Pilgrim and its affliates for all services, and NH Medicaid and Healthy Kids for gynecological services.
For clients who have other insurance coverage, we require that payment be paid in full at the time of service. We will give them a receipt and the client can submit a claim to their insurance company.

FEES

Fees are calculated based on many different factors including time spent with the practitioner, lab work, and complexity of the visit. Please call (603) 436-7588. Fees for services will be determined at the time of appointment. We accept Master Card, Visa and cash. Personal checks are ONLY accepted for gynecological services from established patients.

STD Fee Schedule

• Routine Screening (Chlamydia, Gonorrhea, Syphilis) $35 - Sliding Scale is available for routine screening
• Herpes Western Blot Serology with typing $60
• Herpes Culture $70
